Twitter has so many funny clever comments about this paper today, I can’t find my way back to the original question...but it is who did the peer review? And the answer from Lancet was...

Hard to believe (or maybe not so hard). Someone commented that when the people who wrote the article also do the "peer review" it's usually called editing.
