ok, so in the video part 1, around 3:18, he says about gout prevention:

cut back on beer
lose weight
stay away from hfcs

my wife doesn't drink alcohol, has never had a sweet tooth, and is below the obesity BMI - though she is overweight. i'm reading the book, but don't see initially anything earth shattering that she can do to stop these gout attacks on her foot.

the only thing that i can think of is for her to go completely lchf, which i don't see happening. she has cut carbs a lot ( mostly rice! ) and as i've posted elsewhere, her A1C at her latest type 2 diabetes appointment fell from 6.4 to 6.1. it was 6.7, before that - so she's headed in the right direction.
