I´m using the carbohydrate counter, what does carbohydrates by difference mean? Is that the count I should aim to be under 20 on?

The 'by difference' is how the carb content in foods is usually determined in the U.S. The fats and proteins and ash are measured and everything else is assumed to be carbs. The fats, protein and ash amounts are subtracted from the total and the difference is assumed to be carbs.

Most low-carbers subtract the fiber amounts from the listed carbs levels to get a net carb amount.
