Well, my own theory is it starts because the body can't cope with constantly being dosed with high blood sugar glucose. Its like running your car engine at too many RPMs for too long, taint good for it.
The symptoms usually start as high BP, fat accumulation around the stomach (and internal organs) and rising morning blood glucose and the body starting to be unable to keep blood glucose in safe area after eating. Increased triglycerides is another symptom. Middle aged people are quite often the poster children for this although young people are starting to get it too: Fat around the middle, flushed faces.
