There is some bit of carbohydrate in cream, but I think there are other problems that might make more sense than the bit of carbs in heavy cream. People usually take heavy cream in coffee or tea--in which case, rather than eating because they're hungry, they may be taking in substantial calories just to feed a caffeine habit. If I take heavy cream in my coffee without measuring it, I find I end up getting most of my calories from coffee. Another major use of heavy cream is as a dessert, with artificial sweetener or with berries etc., there's another problem, it takes more sweetened cream in my experience to satisfy me than unsweetened.

In the long term, probably not. In the short term, especially early on when appetite suppression from low carb and IF is likely to be at its highest, though, I wouldn't worry about it. And if those calories include cream, and your appetite is still that low, I wouldn't worry about the cream for now, either.
