I am type 2,now on Glucovance 5/500 2Xdaily,Iam thinking of
asking my doc for 2 separate rx one for Gliberide one for
Metformin,or possibly Prandin & metformin.The reason is I
could tailor my meds to my needs.Any ideas on this ,or is this
dumb. Does anybody know what the max amount per day of each of
these meds.Right now I take 1 Glucovance at breakfast and 1 at
dinner,about 2-3 hrs after lunch I get a little
hypo-70s-60s,but after dinner sometimes I am a little high 2-3
hrs after. Thanks

I take Metformin with Prandin. I like the control I can have
with the Prandin. I'm not sure of the max dose for the Prandin
(I thinks it's 4mg before a meal) but I now take 1mg before
meals. If my readings are good or I've been having hypos the
doc said I can cut it back or eliminate for that meal. I like
how I can customize it to how I am doing that day. It is also
nice because it can allow me to up my carbs for certain meals
without too many adverse effects. I take 1500 of Metformin.
I've had pretty good results so far. No it is not a stupid
idea. As long as you have no problem remembering to take the
Prandin before meals I think it's a good idea. c

This is a superior plan. Sounds like you have been reading
the latest medical sites.
The standard therapeutic dose of metformin is thought to be
1500 mg/day for most people. Maybe your 2 x 500 = 1000 mg/day
are doing a good job, maybe not. The maximum dose is about
2500 mg/day.
Prandin gives you an insulin boost when you need it, not all
day including times when you don't need it. And, best of all,
you get to decide based on your experiences testing your own
blood when those times are.

I take metformin, 1000 2x a day and Prandin, .5mg (that's 1/2)
before meals if I feel I need it. In your case, you might find
you need no Prandidn before breakfast or lunch, but at dinner
it helps. You might also look at how much carbohydrate you are
eating at dinner. Is it more than at lunch? You might also try
just reducing the dinner carbs a bit and see how that works.
Separating your medication rather than using a combination is
always a good idea, as the dose of each can be easily varied.
Prandin or Starlix are both short acting insulin pushers and
don;'t make you hungry all day long. I also personally think
that they put less of a strain on the pancreas.
I used to take Prandin with every meal, but found I had to
feed too many hypos, so my Endo gave me the ability fo use it
as I felt it ws needed-say going to a party or restaurant
where I can't control the kitchen, or at holiday times, if my
control is less than perfect. It will not help with a real
carbo pig-out,(for me, anyhing over 45 grs at a meal) but you
can have just a bit more without a spike.
I think you ae on the right track.

One more thing to add to the other posts. Glucovance is a
proprietory med. By buying generic metformin you will save
some money, which will then help cover the cost of the extra
separate med. I think it's a good idea, myself.

Since Glucovance is a combination drug, many people see it as
preferable to take the separate drugs in their appropriate
doses. Of course it means you have to take two (or more) pills
a day, but if that doesn't bother you, splitting the drugs may
be preferable. As it stands, if you want to increase the
metformin, for example, you can't do that without also
increasing the glyburide.
It may also turn out that if you increase the metformin you
could do without the glyburide portion at all. There may be
advantages to this since metformin increases your insulin
sensitivity and glyburide forces your pancreas to produce
additional insulin. Metformin is often seen as an anti-heart
attack drug so it has that advantage as well. It is also
unlikely to produce hypos.
