Finally did my test for my new bolus insulin + Symlin (amylin). The Novolog is a pen (1 unit increments) and to stay on the safe side symlin says cut insulin in half... Therefore, instead of 15 g carb meal I had to eat 30 g carb for the test. The story follows:

He carefully weighed out everything and popped the insulin...

In 15 mins, he popped the Symlin and began eating...

He measured glucose a lot...

BG went to 150 - a new RECORD for the past month!!!

It has taken him 3 days to get his numbers back in line. End of story.

... since I wanted to see the BG profile I didn't re-bolus (if that is a word) at 150 (even though I was at 2.5 hours by this point).

As I think Symlin is beneficial to weight loss (and minimizing BG peaks) I would like to use it, but I have been back to my regular diet and will stay there for the foreseeable future. I don't think I can use Symlin with just my basal Lantus but would like to hear from others that may have experience, pointers, or recommendations using it.

Other comments, questions, or criticisms are also appreciated.

TIA,

P.S. Of course, I will ask my DCE as well next week. She is a Type 1 on a pump but not totally on board with Bernstein - i.e., she has never read his book. I still like her as she has not been the least bit judgemental re: my diet.

FDA presently requires injection of a bolus and Symlin with meals but I found an overview of a study that used it effectively with Lantus and no bolus... comments look pretty encouraging re: A1c and weight, so.... nevermind. In a nutshell

http://www.emaxhealth.com/23/13315.html

"We are encouraged by the results from this study investigating the use of SYMLIN with an established regimen of basal, or long-acting, insulin," said Matthew C. Riddle, M.D., Professor of Medicine, Oregon Health & Science University. "Improved glucose control often is accompanied by weight-gain or increased hypoglycemia, but with this approach, neither of these problems occurred while glucose control improved."

Others comments or references still appreciated.
